To book a cosmetic surgery date with Dr Graham after your in person consult, we require a non-refundable deposit of $1,000. This will hold your surgery date. This amount will be applied to the surgery cost balance. Full payment is due 6 weeks in advance of surgery date.It will be up to you to ensure that payment is made in full and on time. If you miss the payment due date you will lose your deposit.
Payments can be made by E-transfer, bank draft, or credit card.
Payment in full is due 6 weeks in advance of surgery. If you wish to cancel surgery prior to 6 weeks in advance and have not paid in full, you will forfeit your deposit or amount paid less $1,000 non-refundable deposit.
Once surgery is paid in full your surgery is confirmed and can no longer be rescheduled. Any changes will be deemed as a cancellation subject to our cancellation policy.
If you cancel 4 weeks before surgery 50% of total fee will be refunded.
LESS than 4 weeks before your surgery no refund will be given.
If your surgery is cancelled by the surgical suite or this office, then you can either rebook your surgery or get a full refund.
Exception will be made with the documented loss of a loved one or with a note from another physician (GP) for health-related cancellation reasons.
Refunds will only be made at the request of the patient and will be paid to the patient.
